katikat"A Good Man Is Hard To Find" by kimonkey7 is a fantastic gen and very long (21 chapters) fanfic in which Dean goes through a heroin withdrawal. There is some other plot there too of course, explaining how he got addicted, and there is a monster to kill too, but the process of him going through the withdrawal with Sam's help is the most outstanding part of the story. It's a very detailed description and it's obvious that the author did a lot of research when writing this story. Amazing stuff that kept me up until 2 AM to finish it!
"He Ain't Heavy, He Is My Brother" by EFW is another gen story with lotsa hurt!Dean stuff. It's set shortly after "In My Time of Dying" and the Clown!episode (sorry, I don't remember the title of it now). The boys go through various stages of grief and while stuck in the Roadhouse with Ellen and Ash during a huge storm, they have to deal with Dean having appendicitis. There are two very adorable original characters - the 95-years-old Dr Bates and his lovey wife Betty - who help Sam perform an appendectomy on Dean to save his life. Another long story (11 chapters) and it's obvious that the author did a lot of research too. Very engaging. I spent the whole evening reading it because it kept me on the edge of my seat!
